Your gut microbiome — the trillion or so bacteria, fungi, and other microorganisms living in your digestive tract — is not a wellness trend. It is a legitimate physiological system with documented effects on hormone metabolism, immune function, mood, and inflammation. And it changes in perimenopause in ways that matter.
The Estrobolome: Your Gut’s Role in Estrogen Metabolism
There is a specific subset of gut bacteria called the estrobolome — bacteria that produce an enzyme called beta-glucuronidase, which reactivates estrogen that has been processed by the liver for excretion. When the estrobolome is functioning well, it recirculates appropriate amounts of estrogen back into the bloodstream. When it is disrupted, estrogen metabolism becomes dysregulated — either too much estrogen is recirculated (associated with estrogen-dominant conditions) or too little (compounding the already declining estrogen of perimenopause).
The estrobolome is sensitive to diet. A diet rich in fibre feeds the beneficial bacteria that support healthy estrogen recirculation. Alcohol and highly processed foods disrupt it. Antibiotic use disrupts it substantially. The gut-hormone axis is real and the lever you have most control over is what you eat.
What Changes in Perimenopause
Several studies have found that gut microbiome diversity changes around the menopausal transition — specifically, diversity tends to decrease. This matters because microbial diversity is the primary marker of gut health: a more diverse microbiome is associated with better metabolic health, stronger immune function, lower inflammation, and better mood (via the gut-brain axis).
Bloating, constipation, and digestive changes that arrive in perimenopause without any apparent dietary cause are frequently microbiome changes. Estrogen affects gut motility directly, and its decline changes how quickly food moves through your digestive system.
The Gut-Brain Axis and Mood
Approximately 90% of the body’s serotonin is produced in the gut. The vagus nerve carries constant bidirectional communication between the gut and the brain. Gut bacteria influence neurotransmitter production — including GABA, dopamine, and serotonin. A disrupted microbiome can directly affect mood, anxiety, and cognitive function.
For women experiencing new anxiety, low mood, or brain fog in perimenopause without a clear explanation, this gut-brain connection is worth knowing about. It doesn’t replace addressing the hormonal cause — but it’s another lever, and the dietary interventions that support it are not complicated.
What Actually Helps
Fibre — more than you’re probably eating. 30g of fibre per day is the target. Most women are getting 15-18g. Fibre feeds beneficial gut bacteria (prebiotic effect) and supports regular bowel movements. The most effective dietary change for gut health is simply eating more plants — vegetables, legumes, fruits, whole grains — and aiming for as much variety as possible.
Fermented foods with live cultures. Yoghurt (with live active cultures), kefir, kimchi, sauerkraut, miso, tempeh — these introduce beneficial bacteria directly. The research on fermented foods specifically for the microbiome has strengthened significantly in recent years. A Stanford trial found that a fermented food diet increased microbiome diversity more effectively than a high-fibre diet alone.
Polyphenols. Polyphenols — found in olive oil, berries, dark chocolate, herbs, tea, and coffee — are metabolised by gut bacteria into bioactive compounds with anti-inflammatory and prebiotic effects. The gut microbiome is literally what makes polyphenols useful. This is why the Mediterranean diet, which is rich in polyphenols, consistently shows microbiome benefits.
Reduce ultra-processed foods. Emulsifiers in ultra-processed foods — ingredients you cannot picture in their natural state — have been shown to disrupt the gut mucosal lining and reduce microbial diversity. This is not about clean eating as an ideology. It is about specific ingredient categories with specific documented effects on gut bacteria.
Be strategic with alcohol. Alcohol directly disrupts the gut microbiome, increases gut permeability, and impairs the estrobolome. For women in perimenopause already dealing with hormonal dysregulation, this is worth weighing seriously.
