Hair Thinning in Menopause: What’s Happening, What Helps, and What to Stop Buying

You noticed it in the shower drain first. Then the brush. Then the pillow. Or maybe it was more gradual — a part that got wider, a ponytail that got thinner, a texture that changed. Your hair used to be yours, reliably, in a way you could count on. Now it feels like another thing the transition has altered without asking.

Hair thinning in perimenopause and menopause is real, common, and hormonally driven. It is also an area with a large and largely useless supplement industry attached to it. Here is the actual science and the approaches that have genuine evidence.

Why Menopause Causes Hair Thinning

Hair follicles are sensitive to hormonal signals. They have receptors for estrogen, testosterone, and DHT (dihydrotestosterone — a potent androgen derived from testosterone). The hormonal shifts of perimenopause affect all of these systems simultaneously.

Estrogen’s protective role disappears. Estrogen extends the hair’s growth phase (anagen phase) and supports hair diameter and density. When estrogen declines, more follicles shift into the shedding phase (telogen phase) simultaneously, producing diffuse thinning across the scalp rather than the patchy loss associated with other hair conditions.

Relative androgen dominance. As estrogen declines, the ratio of androgens (testosterone, DHT) to estrogen shifts. Even if total androgen levels aren’t elevated, their relative influence on follicles increases. In follicles genetically sensitive to DHT — which is most pronounced at the crown and front hairline — this can produce the pattern similar to female androgenetic alopecia.

Thyroid function. Thyroid dysfunction — both hypothyroidism and hyperthyroidism — causes hair loss, and thyroid problems increase in frequency around the perimenopause transition. Hair loss that is severe, rapid, or accompanied by other symptoms (weight changes, fatigue, cold sensitivity, dry skin) should always be assessed with thyroid function tests, not just attributed to menopause.

Nutritional factors. Iron deficiency is a common and frequently missed cause of hair thinning in women, and ferritin (iron stores) should be checked specifically — serum iron alone can be normal while ferritin is depleted. Zinc, biotin, and protein deficiency can also contribute to hair quality and shedding, though deficiency-level cases are more common than the supplement industry suggests.

What Actually Helps

HRT. Addressing the estrogen decline that is driving the shift in hair follicle behaviour is the most direct intervention. Many women report stabilisation or improvement in hair thinning on HRT. The evidence is less robust here than for hot flashes or sleep, but the mechanism is sound. Micronised progesterone rather than synthetic progestogens may be preferable, as some synthetic progestogens have androgenic properties that can worsen hair thinning.

Minoxidil. Topical minoxidil (2% for women) is the best-evidenced topical treatment for female pattern hair loss. It extends the growth phase and increases follicle size. Available over the counter. Results take 3-6 months to assess and require continued use — stopping minoxidil will result in reverting to baseline. Low-dose oral minoxidil (0.5-1.25mg) is increasingly used by dermatologists and appears effective with fewer scalp side effects than topical.

Iron and ferritin assessment. Before investing in any hair supplement, get your ferritin checked. Hair thinning in women with ferritin below 30-40 ng/mL often responds to iron supplementation. This is a simple, cheap, and frequently overlooked fix.

Protein intake. Hair is made of keratin — a protein. Women who significantly reduce protein intake (through calorie restriction, plant-based diets without adequate planning, or simply eating less as appetite shifts in perimenopause) can experience hair thinning that responds to adequate protein. Aim for at least 0.8-1.2g of protein per kg of body weight.

Scalp health. Reducing harsh chemical treatments, heat damage, and tight styles that stress follicles supports the hair you have. Scalp massage has modest evidence for increasing hair thickness through improved blood flow. Not a primary treatment but a supportive one.

Dermatologist assessment. If hair thinning is significant, rapid, or patterned (rather than diffuse), a dermatologist who specialises in hair can diagnose what’s happening precisely and recommend appropriate prescription treatments including anti-androgens (spironolactone), PRP (platelet-rich plasma injections), or low-level laser therapy.

What to Stop Buying

Biotin supplements are the most marketed hair supplement globally and have evidence only for deficiency — which is rare. If you are not biotin-deficient, supplementing with biotin does not thicken your hair, and high-dose biotin can interfere with thyroid and cardiac blood tests, which is clinically relevant when you’re in a health-monitoring window like perimenopause.

Most commercial “hair growth” supplements contain a mix of vitamins and botanicals at doses and combinations that have not been tested in clinical trials. They are expensive and the evidence for them in hormonal hair loss is absent.

Get the ferritin test. Consider minoxidil. Address the hormones. That is the evidence-based hierarchy.
